چکیده

Giordano (1963) demonstrated that nitrogen equilibrium could be achieved in chronic renal failure on a nitrogen intake which was less than the minimal nitrogen requirement for normal subjects. His diet was mainly synthetic, and a practical diet with low-protein foods was first devised by Giovannetti & Maggiore (1964), later modified to suit British palates by Shaw, Bazzard, Booth, Nilwarangkur & Berlyne (1965). This diet contained a large proportion of high-biological-value protein providing all the essential amino acids except methionine in the amounts recommended by Rose, Wixom, Lockhart & Lambert (1955), a methionine supplement and an adequate supply of calories. Since that time there has been considerable controversy about the minimal nitrogen requirement for equilibrium in chronic renal failure. Shaw et al. (1965) recommended 0.26 g of protein per kg/body weight, Giovannetti & Maggiore (1964) 0.3 g and Ford, Phillips, Toye, Luck & de Wardener (1969) 0.5 g. Giovannetti (1966) stated that for the diet to be successful a high caloric intake was necessary. Ford et al. (1969) concluded that caloric intake had no effect on nitrogen metabolism in chronic renal failure. However, since their experimental protocol did not compare the effects of changing caloric intakes on nitrogen balance while on isonitrogenous diets, their nitrogen intake changing over a fourfold range, no firm conclusions can be drawn. We report here the results of an investigation into the effect of caloric intake on nitrogen metabolism in chronic renal failure.

Evaluation of the effect of exercise on serum creatinine and urea nitrogen in hemodialysis patients

Introduction. Chronic renal failure is a progressive disease in which kidney function is impaired, so that the body can not metabolize and maintain fluid balance and electrolytes. Therefore, the present study investigated changes in serum creatinine and urea nitrogen in hemodialysis patients in response to exercise. Materials and Methods. The subjects of this study were dialysis patients who we...

The Evaluation of Dietary Protein Intake by Calculation of Urea Generation Rate in Chronic Dialysis Patients in Kerman, 2002

Malnutrition is a common problem in chronic dialysis patients. There are several methods for nutritional assessment of hemodialysis patients, such as calculation of urea generation rate. As urea is produced from proteins catabolism and there is a linear correlation between urea generation and protein catabolic rate (PCR), PCR can be calculated by considering urea nitrogen appearence during the ...

Lactulose efficacy in reduction of nitrogen products, blood potassium and fluid overload in patients with end-stage renal failure.

Abstract Introduction. Chronic kidney disease (CKD) is a major public health problem that often goes unrecognized until late-stage disease. Patients with chronic kidney disease (CKD) face with uremic toxins and hyperkalemia. Also fluid overload in CKD patients is associated with rapid decline in kidney function. Lactulose is a hyper osmotic agent and as a prebiotic could reduce serum urea and p...

EFFEC T OF HEMODI ALYSIS ON TRACE ELEMENTS IN PATIENTS WITH ACUTE AND CHRONIC RENAL FAILURE

Hemodialysis is being implicated in the development of metabolic disturbances, as complications have been observed and the role of trace metals in their development has been questioned. In 78 renal failure patients who underwent hemodialysis, serum levels of zinc and copper were determined before and after first hemodialysis. Acute and chronic renal failure patients were found to have lowe...
